Pre - admission requirements
Let's get started and get to know each other!
Here is a list of things we need to get started.
1. First we need to have a short telephone interview to make a pre-determination as to whether or not the prospective resident is in any of our client groups and if they are ambulatory or non-ambulatory. If it is determined they meet the minimum requirements, then we can go on to step # 2.
2. Next we need to arrange a meeting to complete a Data Gathering form and our staff nurse will make an initial assesment. This meeting will help us serve the prospective resident better.
3. Medication review /psychosocial assesment and medical history from phyisician.
4.Current TB test
5. Results of a recent phyisical exam and a recent medication review by the residents Dr., we also require that the residents risk behaviors be identified and a psycosocial and a risky behavior assesment be completed prior to being accepted for admission.

Our House I Supportive Home Care: - capacity 1 - male or female
Developmentally Disabled (DD), Mentally Ill (MI), Alzheimers (AZ) Frail and Elderly
Our House II CBRF: - capacity 8 - male & female
Advanced Age (AA), Alzheimers (AZ), Mentally Ill (MI) Developmentally Disabled
Our House III Adult Family Home: - capacity 4 male & female
Developmentally Disabled (DD), Mentally Ill (MI) Emotionally Disturbed (ED)
Our House IV Supportive Home Care: - Capacity 1 Advanced Age (AA) Alzheimers (AZ) Mentally Ill (MI) Emotionally Disturbed (ED)
Clasifications
Our House II & III are State licensed facilities. OH II is an Adlut Family Home capacity 4 male and female and OH II is a Community Based Residential Facility capacity 8 male and female.
OH I and OH IV are Supportive Home Care apartments (SHC) capacity 1
These apartments are for someone who is relatively independent yet yet needs some help throughout the day with daily living.
